Assessing the Impact of the Disability

Reflect on the disability’s impact. Has it intensified or lessened since the last review? Or has it remained consistent?

Anticipating Future Changes

Will the participant be embarking on a new journey, such as starting a job, picking up a sport, or undergoing a specific therapy?

Are there any anticipated shifts in personal circumstances, like moving out or a change in the primary caregiver’s ability to provide support?

Special Considerations for Younger Participants

For participants below the age of seven, specific procedures and contacts come into play, ensuring their unique needs are met.

Monitoring Plan Duration

Vigilantly track the NDIS plan’s start and end dates.

Utilise the NDIS Participant Service Guarantee as a benchmark. Ideally, participants should be reached out to about six weeks before their plan concludes.

Be proactive. If you don’t hear anything by the 3-4 week mark leading to your plan’s conclusion, reach out to the appropriate contacts.

Gathering Essential Reports and Quotes

Ensure you have relevant reports, especially if your current plan involves Capacity Building funding for allied health services.

Procure detailed reports from therapists or providers about progress and future recommendations.

Reflect on how your plan is managed, whether it’s agency-managed, plan-managed, or self-managed. Each reassessment is an opportunity to reconsider and potentially pivot your plan’s management style.

Anticipating the Arrival of Your New Plan

Upon completion of the NDIS plan reassessment meeting, a new plan should replace the old one by its expiration date.

In cases where a new plan isn’t ready by the expiration of the current one, the NDIS will extend the existing plan for up to a year at the same funding level, ensuring uninterrupted support.
